BLOCK 4:
a. Contractor format is acceptable. Submittal shall be electronically using the ANSI ASC X 12 Transaction Set 196, Contractor Cost Data Reporting, Version 3040. The Government will have access, on an as-required basis, to electronic Contractor CCDR data.

b. Reporting levels shall be in accordance with the government approved Contractor Cost Data Reporting Plan (CCDR). The approved CCDR Plan will be provided to the contractor 30 days after contract award. The contractor will map the Contract WBS to the CCDR Plan WBS 60 days after contract award.

c. Critical/major subcontractor (team member) summary level data shall be reported and provided as an attachment.

BLOCKS 10 - 13: Initial submission (hardcopy and electronic) shall be no later than 90 calendar days after contract award. Thereafter, subsequent submissions (hardcopy and electronic) shall be provided as follows:

a. 60 calendar days following completion of the PDRR Phase I Preliminary Design Review
b. 60 calendar days following completion of the PDRR Phase I Critical Design Review
c. 60 calendar days following the ATP2 Decision for PDRR Phase II
d. 60 calendar days following the lethality demonstration
e. Final submission due at contract close-out

Blocks 14 and 15. Submit copy of transmittal letter, without data, to SMC/PK
