LAUNDRY, DRY CLEANING, AND LINEN EXCHANGE
AFI 34-901, 19 July 1994, is supplemented as follows and applies to the 21st Space Wing and all authorized users of the Base Linen Exchange, located in the basement of Building 1164, Peterson AFB.
21. 21 SW Form 395, Linen Control Record. Organizational items and special or distinctive clothing or equipment issued to military or civilian personnel can be cleaned through the Linen Exchange Section (SVMLL), 21st Services Squadron. When items are turned in for cleaning, Linen Exchange personnel complete a 21 SW Form 395 in three (3) copies. They annotate the quantity and description of each item turned in. Linen Exchange keeps the original, the second copy goes to the contract laundry facility to be affixed to the customer's order, and the last copy is given to the customer. When items are returned to the customer, the customer signs the original verifying receipt. The Linen Exchange Section keeps the original 21 SW Form 395 for one year.
